This waiver must be filled out in order to be eligible for the Millon Events Summer Invitational.

WAIVER TERMS AND CONDITIONS:

Millon Lacrosse Events has reserved a place for your child (“the participating athlete listed”) in a Millon Event (Summer Invitational). The undersigned parent or legal guardian hereby agrees to enroll the participating student based on the terms and conditions stated herein. The undersigned parent(s) or legal guardian(s) are hereinafter collectively referred to as “I” or “me.”

I. MEDICAL INSURANCE INFORMATION:

I understand that every participating student is required to have health insurance coverage that provides an appropriate level of benefits befitting a participant in a contact sport. I affirm that my child has an appropriate level of coverage.

II. WAIVER, ASSUMPTION OF RISK, AND RELEASE OF LIABILITY:

I recognize that participation in this Millon Event places the participating student at physical risk, as there is an inherent risk of injury when participating in a contact sport. I agree to assume that risk. Although Millon events directors have taken reasonable measures to protect the participating student, I realize accidents, illness, and injuries may still occur. I hereby waive, release and forever discharge Millon Events, the Venue, all coaches and staff, representatives, employees, contractors, successors and assigns of any of the preceding of and from any and all rights and claims for damages to person and property which may be sustained or occur during participation in Millon Event by the participating student whether paid damages, injury or loss are due to negligence or not.

III. MEDIA WAIVER

I understand that Millon Events occasionally uses photographs and/or video of gameplay of Millon Events for promotional purposes. I grant Millon Events permission to use the participating student’s name, image, and identity in any program for the promotion of Millon Events including written materials, website materials, video, or other forms.

SPORTSMANSHIP – Players, coaches, and spectators should exhibit the highest level of sportsmanship at all times. Coaches and officials should be partners in providing the best possible lacrosse experience for youth players. Unsportsmanlike conduct by coaches and/or players and/or spectators degrades the experience of youth players, discourages people from becoming or continuing as officials, and erodes the integrity and appeal of the sport. Therefore, unsportsmanlike conduct will not be tolerated. Obscenities need not be used in order for language to draw a penalty. Tone, intent, and body language can all contribute to unsportsmanlike conduct. US Lacrosse expects officials to enforce the Unsportsmanlike Conduct rules without hesitation.

FAMILY BEHAVIOR – Any parents, friends, grandparents, etc acting in an inappropriate, crude, or unsportsmanlike manner is subject to ejection. Said ejection could result in removal from the grounds at any host facility at the discretion of the tournament director. This includes but is not limited to foul language, taunting other teams or their coaches, taunting other parents in the stands, and taunting or berating an official. This is a great event for the teams that should in no way be negatively impacted by parents and their behavior. Please make your parents aware of this rule prior to the event, as there will be no “second chances” if they were not made aware of the rule. If the behavior continues after sanctioning takes place, the team could face penalty time enforced through the refs or disqualification from the event.

Acknowledgment of Risk & COVID-19 Warning & Disclaimer

I, in my legal capacity as the parent/guardian of the minor named below, do hereby acknowledge and agree that participation in Millon Events activities comes with inherent risks. I have full knowledge and understanding of the inherent risks associated with Millon Events participation, including but in no way limited to (1) athletic injuries, and (2) illness, including exposure to and infection with viruses or bacteria. I further acknowledge that the preceding list is not inclusive of all possible risks associated with Millon Events participation and that said list in no way limits the operation of this Agreement.  Coronavirus, COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. Federal and state authorities recommend social distancing as a means to prevent the spread of the virus. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in Millon Events could increase the risk of contracting COVID-19. Millon Events in no way warrants that COVID-19 infection will not occur through participation in Millon Events.

Waiver, Release, Indemnification & Covenant Not to Sue

In consideration of my child’s participation in Millon Events, I, the parent/guardian of the minor named below, agree to release and on behalf of myself and the minor named below, my heirs, representatives, executors, administrators, and assigns, HEREBY DO RELEASE Millon Events, its coaches, directors, employees, agents, representatives, and insurers (“Releasees”) from any causes of action, claims, or demands of any nature whatsoever including, but in no way limited to, claims of negligence, which I, the named minor, my heirs, representatives, executors, administrators and assigns may have, now or in the future, against Millon Events on account of personal injury, property damage, death or accident of any kind, arising out of or in any way related to the participation in Millon Events events whether that participation is supervised or unsupervised, however, the injury or damage occurs, including, but not limited to the negligence of Releasees.  In consideration of the named minor’s participation in Millon Events, I, the undersigned parent/guardian of the named minor, agree to INDEMNIFY AND HOLD HARMLESS Releasees from any and all causes of action, claims, demands, losses, or costs of any nature whatsoever arising out of or in any way related to the named minor’s Millon Events participation.

I hereby certify on behalf of myself and the named minor that I have full knowledge of the nature and extent of the risks inherent in Millon Events participation and that I, on behalf of myself and the named minor, am voluntarily assuming said risks.  I understand that I and the named minor will be solely responsible for any loss or damage, including personal injury, property damage, or death, the named minor sustains while participating in Millon Events activities and that by signing this agreement I, on behalf of myself and the named minor, HEREBY RELEASE Releasees of all liability for such loss, damage, or death. I further certify that the named minor is in good health and has no conditions or impairments which would preclude his/her safe participation in Millon Events activities.  I further certify that I am of lawful age (18 years or older) and otherwise legally competent to sign this agreement and that I have the legal capacity to act as the parent/guardian of the named minor.  I further understand that the terms of this agreement are legally binding and certify that I am signing this agreement, after having carefully read it, of my own free will.
